Below is some information regarding your upcoming clinic:
1. Insurance billing and payment process information:
- Bring your insurance card or a front/back copy of the card
- You will have a $0 copay with Health Choice, Community Care, BlueLincs, Medicare and Global Health insurance
- For staff that have prescription insurance coverage other than the above, we will bill your insurance, although a copay may apply.
- Staff who are not insured can get vaccinated for $25.
2. Please bring a copy of your Driver's License or State issued ID.
3. Schedule an appointment with us using the link below to help us know which additional vaccines to use and to prevent wait times.

*Please have employees print and bring their completed forms to the clinic. Additional forms will be available at the clinic for those that did not sign up on the link.
5. Family members are welcome to attend the clinic (Please no children under the age of 4)
6. Wear a shirt with loose fitting sleeves the day your clinic is scheduled.
7. Other shots we will also have available:
- Covid-2019 Vaccine (Primary doses)
- Covid Bivalent Booster (Moderna & Pfizer)
- T-DAP (Tetanus, Diphtheria & Pertussis)
- Prevnar 20 (Pneumonia)
- Pneumovax 23 (Pneumonia)
- Shingrix (Shingles)
- Vitamin B-12 shot, not covered by insurance, $10.00 cash/check/card (including HSA benefit).
